In order to complete your social security registration and be able to subscribe FREMAP's coverage for financial compensation for temporary disability due to common illnesses, work-related accidents and illness coverage and coverage for termination of activity, you must fill in form TA.0521 to register for the Special Regime for Self-employed Workers, specifying your mutual insurance company (FREMAP 061).

1. Download the termination request here.

2. Send an email to your current health insurance provider (* see mailboxes) with the request for cessation signed with the self-employed person's digital certificate before 1 October. You can also submit the request in duplicate to the previous mutual insurance company, or send it in any other verifiable way. 

3. Request the change of mutual insurer for the self-employed via RED System or SEDESS, which will take effect on 1-1-2024: here.  

4. Forward to FREMAP the email sent to your current mutual insurance company, informing us of the date on which you requested the change in the Red System or SEDESS. 

Keep in mind that the change will be effective starting on 1 January of the following year.
